Optimize Your Community: Improve Population Health through Screenings.

This Community of Practice will offer sessions on topics including the importance of early health screenings in saving lives, the role of outreach in communicating the value of preventive care, the barriers patients face in accessing screenings, and best practices for strengthening referral and follow-up systems. Participants will also have opportunities to share challenges and successes with other health centers in a supportive environment.
