NACCHO Cardiovascular Health Community of Practice: Peer-Learning Sessions
Join NACCHO’s Cardiovascular Health Community of Practice (CoP), a peer-learning network for local health departments working to prevent cardiovascular disease and reduce key risk factors. The CoP provides a space to share strategies, learn from peers and experts, and exchange practical approaches to advancing heart health in local communities.
The next Cardiovascular Health CoP peer-learning call will take place on July 21 at 1:00 PM ET.
This session, Expanding Access to Hypertension Prevention and Control: Local Health Department Partnership Models in Action, will feature two local health departments sharing how they moved cardiovascular prevention beyond traditional public health and healthcare settings to reach residents where they are.
Hear directly from :
• Porter County Health Department (IN) – Learn how partnerships with libraries, community organizations, and the YMCA helped expand access to blood pressure monitoring, increase awareness of cardiovascular risk, and connect residents to prevention resources in trusted community spaces.
• Los Angeles County Department of Public Health (CA) – Learn how partnerships with FQHCs supported hypertension quality improvement efforts through team-based care, health information technology, social needs integration, and peer learning across clinics.
Participants will explore:
– Strategies for building and sustaining partnerships across community and clinical settings
– Approaches to expanding access to blood pressure monitoring and prevention resources
– Lessons learned from implementation hypertension-focused initiatives
– Considerations for sustainability and adapting similar approaches locally
