Navigating Care – Enhancing Patient Support Services Community Of Practice
Health Outreach Partners (HOP) invites health center staff to a four-part virtual series on patient support services to address healthcare access barriers. The Association of Asian Pacific Community Health Organizations (AAPCHO) will lead the third session on strengthening the CHW workforce, and the final session will highlight promising practices for integrating supportive services.
