Join ASTHO and NACCHO for an informational webinar to discuss a Request for Proposals that will fund up to 50 communities at $50,000 each to evaluate their work to advance health equity by addressing social determinants of health (SDoH). The
purpose of this funding opportunity is to identify and learn from multi-sector coalitions with a demonstrated history of addressing the SDoH outcomes related
to chronic disease conditions, focusing on the built environment, clinical-community linkages, food insecurity, social connectedness, and tobacco-free
policies.
